How to apply?

An application for loss of DEP as oer the Format / damage to DEP as per the Format submitted on the letterhead of the company to the Asstt. Traffic Manager, Permits duly signed by the authorized signatory

Where to Apply?

Applicants will have to submit application at the office of Asstt. Traffic Manager, Permit Section, Shramik Bhavan, Carnac Bunder, P. D’Mello Road, Mumbai 400009

How to make payment?

(i) In case of loss of Smart Card : Rs. 300/- per person (non refundable) (inclusive of penalty of Rs. 150)  

(ii) In case of damaged Smart Card : Rs. 150/- per person (non refundable)  

to be paid at Permit Section
