The application to be sent to The Deputy Conservator, Mumbai Port Trust, Port Bhavan, 1^st Floor, S. V. Marg, Mumbai – 400 001 during working days from 10.30 a.m.  to 11.30 a.m. and 02.30 p.m. to 03.30 p.m.

The party to submit application on their letter head for visit to vessels at Stream / JD & PP must indicate :            

a)  Period from ___________  to ______________

b)  Name of Vessel to visit

c)  Place of Vessel

d)  Reason for visit

e)  List of persons with full name, age and designation
