Note :- In all cases Photograph is must.

1.

Application to be submitted on letterhead of party 

2.

Dock Entry permit issued by the Docks Department.

3.

Authority letter is required in case Oil Companies alongwith Photocopy of Identity Card in case of their permanent employees.

4.

Authority letter is required from Oil Companies alongwith parties application in case of work allotted to private firms.

5.

Authority letter is required from Chief Engineer / Chief Mechanical Engineer,  MbPT in case of tender work.

6.

'No Objection Certificate' from NOC from CID, Police Station or Copy of Passport is required in case of Stream passes for launch crew members.

7.

Families of Crew members of visiting Ship should produce e-mail / fax from the Vessels Master / Ship Security Officer that the person would be allowed on  board. Only immediate family members limiting to three (3) in number are permitted.
